Dashboards

What are these numbers?

There are three dashboard pages based on the Community Outcomes Report:

  • Inflow/Outflow shows the distinct count of clients by their most recent HIFIS status transition. Note that previously active clients may be included in inflow if an active clients status changes to another active status (i.e. signing a new consent). This acts as a snapshot of most recent HIFIS activity for the given time period. Note that over longer periods of time (greater than 90 days) can lead to less accurate transition data, as clients who are no longer active will be counted alongside current clients.
  • Historic Data shows the inflow and outflow status transitions for the previous year. Note that for the clients with "Unknown" status are counted in outflow, and "Housed" is not counted in outflow. There is a separate graph that shows the "Housed" counts each month. To remove "Unknown" from the outflow you can exclude it from the filters.
  • Demographic Snapshot shows the distinct counts of clients identified in HIFIS over the specified time period.


Note that when you select a time period, the report only accounts for their most recent interaction in the system for that time period (so as to prevent duplicate counts).


Data goes back to 2023, and reports with earlier dates will run and the numbers for those years will be counted as 0.

Glossary

  • Public Institution: Refers to clients who are living in facilities provided or funded by the government. These can include shelters, group homes, correctional facilities, or hospitals.
  • Chronically Homeless:  Individuals who have been homeless for an extended period, typically more than 6 months or 180 days, marking them as chronically homeless.
  • Transitional:  Refers to a temporary housing situation where clients are in the process of moving from homelessness or unstable housing to more permanent and stable living conditions. This often involves short-term accommodation solutions like transitional housing programs, which provide support and resources to help individuals and families stabilize and prepare for long-term housing.
